2. Eligibility for Treatment

Before purchasing any medications or services from NSO-LAB, it is your responsibility to ensure that you are eligible to receive these treatments in your jurisdiction. We reserve the right to decline service to anyone not meeting the necessary criteria.

Important! Both Semaglutide or Tirzepatide medication are not insulin and should not be used if you have type 1 diabetes or if you develop diabetic ketoacidosis. Semaglutide or Tirzepatide should not be used with any other GLP-1 medications.

RISK OF THYROID C-CELL TUMORS. If you or any family members have been diagnosed with Multiple Endocrine Neoplasia Syndrome Type 2 or Medullary thyroid cancer you should neither take Tirzepatide nor Semaglutide. If you have ever had pancreatitis consult with your medical provider as using Tirzepatide or Semaglutide can increase the risk of developing pancreatitis.

10. Booking an appointment

By booking an appointment with us, you agree and understand the following: when you book an appointment with us, there is a charge of $100 associated with the reservation.

Cancellation Policy: We kindly request that you cancel your appointment at least 24 hours in advance. Failure to cancel or postpone your appointment in a timely manner will result in the forfeiture of your $100 payment

No-Show Fee: Not showing up for your appointment without prior cancellationwill result in the forfeiture of your $100 payment.

Please be aware that this policy is in place to ensure the efficient scheduling of appointments and to provide fairness to our clients and staff. Thank you for your understanding and cooperation.

11. Return and refund policy

11a. Information

NSO-LAB Weight Loss Clinic is a medical clinic providing the latest safe and effective treatments for weight loss. NSO-LAB Weight Loss Clinic is a cash clinic that does not accept medical insurance.

We provide full paperwork for patients to seek reimbursement for any services and prescription medications that may be covered by the patient’s insurance. Payment for services is the responsibility of the patient regardless of insurance coverage. This return policy constitutes the only conditions applying to the return of products and services, no other policy to this effect exists, and this policy supersedes all prior policies.

11b. General

  1. NSO-LAB Weight Loss Clinic does not offer refunds on accounts with outstanding claims or balances; or where upcoming services are scheduled.
  2. NSO-LAB Weight Loss Clinic does not allow the transfer of services or treatments to other individuals or other clinics.
  3. Submission of the returned product does not constitute acceptance of the return NSO-LAB Weight Loss Clinic reserves the sole right to determine whether the products or services qualify for replacement/refund. NSO-LAB Weight Loss Clinic also reserves the sole right to refuse a return as it deems appropriate.

11c. ITEMS DEEMED RETURNABLE

NSO-LAB Weight Loss Clinic will not issue a refund, but will promptly replace items meeting the following criteria:

  1. Recalled products.
  2. Incorrect shipments
  3. Damaged product due to shipping carrier

11d. ITEMS DEEMED NON-RETURNABLE

NSO-LAB Weight Loss Clinic does not accept the return or refund of:

  1. Services provided or performed; including but not limited to physician visits and patient training. If initial patient training was unsuccessful. NSO-LAB Weight Loss Clinic will provide additional training at no cost to the patient, as needed.
  2. Products or services not purchased directly from NSO-LAB Weight Loss Clinic; NSO-LAB Weight Loss Clinic reserves the right to destroy non- NSO-LAB Weight Loss Clinic products shipped as a returned product and to charge the patient for the cost incurred to process and destroy the non- NSO-LAB Weight Loss Clinic product
  3. Prescription medications, including but not limited to any prescription written by a licensed physician.
  4. Damaged products, including but not limited to missing or damaged labels, repackaged product, opened product, and product otherwise not in its original form.
  5. Mishandled products, due to patient’s improper handling or storage of the products.

11e. PROCESS FOR RETURNS/REFUNDS

NSO-LAB Weight Loss Clinic will only accept the return of product for consideration of refund, if applicable, under the following conditions:

  1. For recalled products:
    1. In the event of a recall initiated by NSO-LAB Weight Loss Clinic or any other pharmacies used by NSO-LAB Weight Loss Clinic or government agency, NSO-LAB Weight Loss Clinic agrees to pay reasonable out-of-pocket costs for costs incurred by the patient.
    2. NSO-LAB Weight Loss Clinic will provide the patient with instructions on how to return any recalled product.
  2. For incorrect shipments, and products damaged due to shipping carrier:
    1. NSO-LAB Weight Loss Clinic must be notified within seven (7) days of the incorrect or damaged shipment. Please contact the office at (561) 698-1919
    2. Mail the product via USPS to NSO-LAB Weight Loss Clinic
    3. Damaged products will be replaced, not refunded, within seven (7) business days after receipt of an approved return.

11f. CANCELLATION OF FUTURE PURCHASES

If the patient chooses to discontinue participation in the monthly subscription service to receive medication, the patient must inform NSO-LAB Weight Loss Clinic in writing, at least 30 days prior to the charge date for the next shipment. Failure to do so will result in the charge being completed, and no refund will be issued.

11g. CHARGEBACKS INITIATED BY THE PATIENT

NSO-LAB Weight Loss Clinic takes patient care very seriously, and our clinics strive to provide the highest quality care to every individual. In the event the patient is not satisfied, we ask to please refer to this document rather than initiating a chargeback with one’s credit card company. If a chargeback is initiated NSO-LAB Weight Loss Clinic will use all documentation available to refute the chargeback including patient acknowledgment of the refund policy.
